Leeway runs four programs from 40 Albert Street in New Haven, and they use two different intake routes. Skilled nursing and the Leeway Residential Care Home go through the Admissions Office. Permanent supportive housing goes through Connecticut's 2-1-1 and the Coordinated Access Network. This page tells you which door is yours and who to call.

Leeway is not an emergency shelter and does not offer walk-in or same-night beds. We provide permanent supportive housing for people living with HIV/AIDS, available by referral through Connecticut's Coordinated Access Network. If you need a shelter or a bed tonight in New Haven, call 211.

Which Leeway program is the right door?

Skilled Nursing (Long-Term Care): 30 skilled nursing beds with 24-hour clinical care. Who to call: the Admissions Office, (203) 865-0068.

Leeway Residential Care Home (RCH): "Supervised living with medication assistance and case management for residents who no longer need 24-hour nursing." Who to call: the Admissions Coordinator, (203) 865-0068.

Permanent supportive housing: "Long-term, affordable homes across New Haven paired with onsite support services." Who to call: 2-1-1 in Connecticut, and ask for a Coordinated Access Network housing appointment.

How do I apply or refer someone for skilled nursing or the Residential Care Home?

Both programs go through Leeway's Admissions Office. The Admissions Office page states: "Referrals are accepted from hospitals, home care agencies, outpatient clinics, as well as from private physicians." It continues: "An application is completed and sent to the Leeway Admissions Coordinator, 40 Albert Street, New Haven, CT 06511."

For the Residential Care Home, the program's own page states: "Referrals are accepted from hospitals, clinics, private physicians, and community agencies. How do I apply for supportive housing?

Housing referrals go through Connecticut's central intake, not through Leeway's Admissions Office.

  1. Call 2-1-1 in Connecticut.

  2. Request a Coordinated Access Network (CAN) housing appointment.

  3. Mention Leeway during the assessment.

Who is eligible for skilled nursing?

The Admissions Office page states: "Anyone 16 years of age or older, who has HIV/AIDS, and who needs 24-hour skilled nursing services is eligible for admission."

The same page states two conditions that apply to that route. "All applicants must have a source of payment available to cover the cost of care at Leeway." And: "Placement on the waiting list does not guarantee admission to Leeway; it indicates only that the potential resident will be considered for admission."
