We provide compassionate, state-of-the-art short-term, long-term, hospice and respite skilled nursing care to people living with HIV/AIDS.
Here are some frequently asked questions regarding admissions:
Who can be admitted to Leeway?
Anyone 16 years of age or older, who has HIV/AIDS, and who needs 24-hour skilled nursing services and/or supervision is eligible for admission. In addition to nursing services, Leeway also offers IV therapy, wound care, physical therapy, occupational therapy and speech therapy, as well as supervision with medications and treatments.
How does one apply to Leeway?
Referrals are accepted from hospitals, home care agencies, out-patient clinics, as well as from private physicians. An application is completed and sent to the Leeway Admissions Coordinator, 40 Albert Street, New Haven, CT 06511. All necessary medical information and required tests must be completed in order to be placed on the facility waiting list. Placement on the waiting list does not guarantee admission to Leeway; it indicates only that the potential resident will be considered for admission. If all requirements are met, the applicant is admitted in the order in which he or she appears on the list.
Who decides who will be admitted?
All potential applicants will be reviewed by the Leeway Admissions Committee which consists of the Admissions Coordinator, Administrator, Director of Nursing Services, Nurse Practitioner, a member of the nursing staff, and the Social Services Director. If deemed necessary or appropriate, the committee may require an interview with, or examination of, the potential resident, or require the submission of additional information by the applicant's personal physician.
What is considered in making a decision regarding who will be admitted?
The Admissions Committee considers whether or not the level of services required by the applicant can be provided at Leeway at that particular time, taking into account the resources, personnel, equipment and services available at Leeway and the medical and psychosocial needs of the applicant. Also taken into consideration are any communicable diseases that may be present and the applicant's need for isolation facilities. In addition, the applicant must have available a source of payment to cover the cost of care at Leeway. Sources of payment include, but are not limited to, Medicaid, Medicare, private insurance, or other sources of payment.
